Module 2 – Assignment – Organizational Structures and Functions
Based on the data gathered, the general picture you have obtained, and the results of the survey you will now focus on the organizational structure and functions for the proposed GAH and Community Clinic.
In order to do so, you will need to define/address the following points:
Note what common services or functions they can share (e.g., blood bank and laboratory services, shared operating rooms, pharmacy, etc.)
Explain the organizational relationships between the structure blocks.
How (if appropriate) will you integrate the services of physicians whose specialty is in geriatric treatment within the GAH /CC?
Address the management aspects, not the clinical ones.
You can choose and explain one of the organizational forms detailed in the following article: Cuellara, A. E. & Gertlerb, P.J. (2006). Strategic integration of hospitals and physicians. Journal of Health Economics. Volume 25, Issue 1, January 2006, Pages 1-28. http://faculty.haas.berkeley.edu/gertler/working_papers/hospital_vi_5_10_02.pdf
Define the mission statement for the GAH and Community Clinic.
Prepare a simple organizational diagram, depicting the organizational structure (blocks) of the existing Mission Hospital (MVH), Children’s Hospital (CHOC), and the proposed Golden Age Hospital (GAH), and the community clinic (CC)
What is the recommended hospital size (number of beds)?
What major services/treatments will be offered based on your survey results at the GAH and the CC? At least six detailed services/treatments are required.

Background
Health is one of the main fundamental rights for all persons, as it allows them to be
productive enough to help in the growth of the economy and in taking care of their families.
Aging is a natural process, and with it comes challenges for all sections within the society.
Globally, there has been a gradual improvement in the delivery of healthcare services. Therefore,
there has been an increase in life expectancy, which has led to a rise in the number of the aged
population. It is estimated that by the year 2025 those aged above 60 years will increase to 1.2
billion (Hold et al., 2019). These transitions in demographics require a focus on the healthcare
requirements of the elderly population. However, the unique health needs of the elderly have not
been considered, and most hospitals that are available only continue to manage children and the
general population without considering the individual needs of the aged. The Mission Viejo
Hospital has, therefore, come up with a suggestion of integrating a hospital to cater to the health
needs of the elderly.
There are currently no specific hospitals that cater for the health needs of the aged
population since most of those present are only homes for them and receive consultant health
services from the general hospitals. Clinicians in those hospitals have also not been trained to
meet the specific needs of the elderly and therefore treat them just like the general population.
These populations tend to have a higher rate of developing physical disabilities, chronic diseases,
and mental illnesses. Their health needs can also not be viewed in isolation since they are
coupled with social concerns like being left by their children in such occupations, leaving them
isolated with no one to support them in their daily activities.
3
Golden Age Hospital (GAH)
The medical conditions that are common among the aged population include diabetes,
hypertension, arthritis, fractures, degenerative diseases, heart diseases, cancer, and mental
illnesses. They also have other problems that are not directly related to health but are risk factors
for developing health problems. Such issues include financial constraints, poor knowledge
regarding nutrition, and risk factors for various diseases, and lack of affordable and adequate
health insurance. They also have concerns about accessibility and inadequacies in the healthcare
system within the government. All these factors have a significant impact on their quality of life.
The types of institutions that cater to the health problems of the elderly are nursing
homes. The population found in such homes is varied, although most are taken there due to old
age, even when they have no ailments (Hold et al., 2019). Others are taken there for aftercare
services when they are discharged from various hospitals. Each home usually has around 100
patients. There are no specific special services provided except rehabilitation services for those
who have suffered multiple ailments. Psychotherapy services may also be offered since many of
their families have isolated them. Most of them stay for about three months, but others stay for
life since they have no one to take care of them at home. Those hospitalized in general hospitals
remain until when they recover and are discharged home.

The total population in the Mission Viejo area is 449,026. The senior people who may
need to utilize services of GAH depending on the median age living in each of the municipalities
is 83, 669. The older population in this area is not significant since the median age of people in
the various cities is below 50 years (Stats about all US cities). The target market subjects for the
GAH project are the senior citizens above the age of 50. They are distributed in three
municipalities within the Mission Viejo area, which are San Juan Capistrano, Laguna Woods,
and Laguna Hills.
The individual needs required by this population include social services like support
groups where they can sit with their age mates and share their different life experiences
(Shrivastava, Shrivastava & Ramasamy, 2013). Support groups are essential as human beings are
Golden Age Hospital (GAH)
social beings, and this population has been isolated and therefore needs to socialize. They also
need to be handled by people who are patient with them and who are understanding since some
have dementia and keep forgetting certain things.
The unique health services include the care of those with diabetes to help them with
reasonable control of their blood glucose using drugs as well as diet. They also need specialized
health care for the management of dementia as many of them suffer from the disorder and are,
therefore, at risk of injury and even getting lost when left in their own homes. They also need
physiotherapy services as their musculoskeletal system is affected, and most have difficulties
walking. They are also prone to pathological fractures (Shrivastava, Shrivastava & Ramasamy,
2013). Being assisted in carrying out exercises that they can tolerate would be very helpful to
them. They also need psychotherapy services since most of them get mental illnesses, some of
which are related to depression.
The physician specialization that they would wish to have is cardiovascular-related care
since they suffer from heart conditions, which puts them at risk of inability to carry out their
daily activities. The physician will also deal with problems related to blood pressure and
ophthalmic health issues. The population would prefer a medical center hospital as well as a
community clinic so that they can easily access the health and specialized services even when
they are discharged home. They also need to be linked with counselors within the community
who can offer them services as needed.
